Blue Shield High-Mark, in the State of Pennsylvania, requires DPM's to be Certified in Radiology to be reimbursed for x-ray claims. Steps To Become Certified 1. Become a DPM Member of ASPMA:Download Membership Form and mail with a check to Susan Weeks, PMAC, Membership Chair. Applicant MUST be a member in good standing of the American Podiatric Medical Association. A one-time membership fee of $100.00 must accompany the application. 2. Order DPM Radiology Certification CD: Download DPM Radiology Certification CD Order Form. Fill it out completely. Either mail with a check or FAX with Credit Card Information. 3. Complete the Exam 4. MAIL Answer Sheet back to Sue Hasenour, PMAC, Qualifying and Examining Chair. 5. Upon successful completion of Exam, a Certificate will be issued to the DPM that can be submitted to High-Mark for their requirements.
